Register as a patient
How to register
Registering with our practice is quick and easy. You can download a registration form by clicking the button below. Please fill out this form and send it to the practice via spring.terrace@nhs.net.
To register, you must also send a copy of your ID. This can be a passport, driving licence, utility bill or medical card. Children do not need to do this as long as at least one parent is registered with the practice.
Please also complete and return a copy of the patient survey. You can download this by clicking the button below.
If you are unable to print or e-mail the documents, please collect a paper copy from the site where you wish to be registered.

Usual doctor
All patients are given a named GP. This GP is responsible for your overall care at the practice. They are referred to as your ‘usual doctor’.
You will be informed of your usual doctor when you register. You can check who your usual doctor is with any member of staff. We urge you to see your usual doctor where possible. This is so we can offer you better continuity of care. If they are fully booked, you can ask for another doctor.
We will make reasonable efforts to accommodate requests to change usual doctor.
