Pride in Practice questionnaire

NPC Spring Terrace is currently undergoing certification for a Pride in Practice certificate. This is focused on care for our LGBTQ+ patients. You can find out more about Pride in Practice here.

We are inviting patients to fill in optional information about their gender identity and sexuality. We will use this information to provide better care. For example, if someone has legally changed their gender identity, they may not be invited for a smear test or Abdominal Aortic Aneurysm (AAA) screening.

We want to ensure that everyone receives the right care, at the right time.

Starting with new registrations, we are asking patients if they would like to provide information about their gender identity and sexuality. We are also inviting current patients to add this information to their records.

To provide us with this information, you will need to complete a short form. It should only take a few minutes. You can click the button below to download the form. Once you have completed it, you can return it to the practice by post, in person, or by e-mailing a copy to spring.terrace@nhs.net.

Following this trial period, we hope to add the form to our online registration form.

All information gathered is strictly confidential. It will be added to your private patient record. This will be able to be viewed by both the admin and clinical staff.
